In order to provide online registration we have developed a PDF that you can email to MMR securely with registration and payment information. If you do not wish to register online, you may simply complete this form, print it, sign it, and mail. Please print this page and follow the steps below to complete the online registration process.
1. Open the online registration form.
2. You Must Save This File and Open in Adobe Before You Can Email. Save the form by clicking the “Save” icon or by going to File/Save As, and selecting your desktop (or another location) in the Save in bar to save the form to.
3. Open the document from the new location.
4. Complete the form filling in all appropriate information. (see last page of registration for details on how to complete the form)
Please submit one form for each Rancher and each week attending.
5. Signing for Medical Release & Credit Card: An electronic signature is required to register online.
If you have used an electronic signature before, then sign and go to step 6. If not please complete the following steps
Click in signature field: You may receive a “Document is not Certified” warning, this means that the information in this document is not certified and therefore could have been changed. Please read the information in this document carefully, if you have any question regarding content please contact our office at (814) 664-7673. Click “Continue Signing”
Add Digital ID, select "Create a self-signed digital ID" and click next.
You may then receive a third party validation statement, read and click next.
“Where would you like to store your self-signed ID”, you may choose either option. The “New PKCS#12 digital ID file” will give you the opportunity to password protect your signature if you wish. Select one and click next.
Enter your full name and e-mail address, the organizational fields may be left blank.
Click next if you selected the PKCS#12 digital ID and enter your password.
Click finish.
6. Once the form is completed you may select to e-mail it using a local e-mail program such as Outlook, Thunderbird, or Mail, or you can select save and e-mail via a web based e-mail account, you would then need to send this document as an attachment, or you may print the form and send via mail.
Once we receive your registration via e-mail or mail we will process the registration and you should receive a confirmation via e-mail (if provided) or via mail. If no confirmation is received please contact our office at mmrcamp@mmrm.org or (814) 664-7673 to confirm receipt of your registration.
